The best champs outside of lane to counter Karthus include Kog'Maw, Xayah, and Kai'Sa. These are all great at countering Karthus with decent win rates over 61%. Even if these Karthus counter picks are not used in the same position as you, it's not a good idea to choose him into any of these matchups. They will likely destroy Karthus.

Yet, if the opposite team picks either Vel'Koz or Tahm Kench, Karthus mains should seriously consider picking him, as he easily dominates these champs with win rates over 52% against each ofthem.
